We see patients by appointment only for non-emergency conditions.  Please arrive at least 15 minutes prior to your scheduled appointment time and bring your insurance card.  Any needed co-pay or deductible amounts will be collected at the time of the visit.  We do accept VISA or Mastercard.  Once registration is completed, you will be ready to see the physician.  Afterwards, any needed procedures, lab work, or x-rays will be scheduled by our scheduling staff.

Final Reminders:

1. Bring a list of your current medications including doses.
2. If you cannot keep your scheduled appointment, please notify us 24 hours prior.

If you do not notify us 24 hrs before a missed appointment, you may be charged a no show fee of $50.
