We need your instructions regarding how to release information about test results or other medical information over the phone.  Please review this document and prnt and complete it before your first office visit to save time at check-in.  The form will need to be updated once a year also.

We need you to help us with instructions about how we should leave medical information when we call your home:

• only when you are one the phone,
• on an answering machine at your home phone number,
• to other family members you choose, or
• other individuals you choose.

We need these instructions to release important infor-mation about test results or other medical information that you need to know about in a timely fashion.  Please review this document and print and complete it before your first office visit to save time at check-in.  You will need to compelte and sign a new form once a year to be sure we have up-to-date instructions from you about this important matter.
