Your medical record is the property of Oak Ridge Gastroenterology Associates. With your permission or a signed authorization from you, a copy of your record may be released.

Records will be sent at your request at no cost to other physicians or hospitals. Due to rising operational costs, we charge a fee for making copies of your record for you, or for sending copies of your medical record to insurance companies, attorneys or disability determination offices. This fee is determined by the amount of work involved to prepare your record for mailing and is to be paid in advance of the record being released.

We request at least two business days notice to copy records records to be picked up by the patient. We make every effort to send requested records to those you authorize as soon as possible.
